How to Register
- Download Registration Forms
Download, print and complete both the registration forms below (one per family member). Simply print at home and complete at your convenience, then return the completed forms back to the surgery with 2 forms of identification (ID).- GMS1 Form
- New Patient Registration Form
- In Person
Simply pop in to the practice and complete our registration forms in person.
What identification (ID) do we need?
Acceptable Identification; tenancy agreement, mortgage agreement, bank statement, driving licence, passport, birth certificate etc.
Until you have submitted your ID you will not be registered at the practice.
